Alternate Location Testing Services
When approved by the Disability Services Office (DSO), students can request
to take test in our Adaptive Technology Lab, Room W209j. The procedure is as follows:
- 1. At least one week priorto the test, submit a request.
- 2. You must take the test within the week that the class is scheduled to take the test.
- 3. Fill out and submit to the DSO the Request for Alternate Location Test Taking along with a Schedule Form to request an appointment to take the test and
inform us of your availability to take the test (Copies are on the bulletin
board outside of W209-j on the West Campus or on this Disability Services webpage).
- 4. When you schedule your test, be certain to allow time for extended time, if that
is your accommodation. That means one and a half times the amount of time
the test is designed to take.
- 5. The DSO staff will check the testing schedule and confirm
your appointment in person, via telephone, or via email. The test
will be scheduled as near to the time of your request as possible, depending
on the availability of a proctor and the testing room.
- 6. Retrieve the green reminder form ALTERNATE LOCATION TEST RESERVATION FORM from the DSO
staff to give to your instructor. This is your opportunity to inform the
professor of the day and time of the test appointment. Remind you instructor
to write out any special testing instructions (e.g. open book) on the green
form when they deliver the test to room W209j.
- 7. Be on time to room W209j! If you are late, that may take time away from your test time.
- 8. If you cannot attend at your scheduled time, call the DSO ASAP:(M-W
call Ms. Alexander 857-6844 or R-F call Dr. Apfel 857-7192).
- 9. When finished, leave the test with the DSO.
- 10. Tell your professor that you have taken the test and remind them to pick it up.
